J10 Cahaba GBA: Alabama Medicare

Cahaba Government Benefit Administrators®, LLC (Cahaba GBA) administers Medicare health insurance for the Centers for Medicare & Medicaid Services (CMS) for Jurisdiction 10 which includes the State of Alabama. If you are a provider located in the State of Alabama and need to become an electronic submitter for your Medicare A/B Claims, please follow the enrollment instructions below.

Required Documents for those applying for new Submitter ID's

The following documents are required enrollment documents that must be completed, signed and returned to the Cahaba GBA office prior to initiation of electronic claims submission or inquiry.

Electronic Data Interchange [EDI] Application PART A [for Providers billing directly]

  1. Select your State
  2. New applicants should choose: A new provider to EDI
  3. Enter your Business/Practice or Provider Name
  4. Enter your demographic information
  5. Enter the name and number of the main contact person for your office
  6. Enter your Fax number and E-mail address
  7. Enter your Medicare Number, NPI, and Tax ID
  8. List any additional Medicare PTANs, NPI’s and EIN’s
  9. Select the option Direct using a vendor
  10. New applicants please leave the Submitter ID box blank
  11. Select the option Electronically Directly from Cahaba
  12. Leave the Submitter ID box blank
  13. Skip the section for Billing Service/Clearinghouse unless you are a provider that is using a Billing Service
          a. Providers using a Billing Service: Please enter your Billing Service's information here
    If using a Billing Service please answer the questions regarding usage of the FISS/DDE System
  14. For "Software Vendor" fill in the following:

         c. Contact Name: EDI Support Team
  15. Please complete the "276/277" section if you would also like to apply for 276/277 (claim status) transactions
  16. Complete the last page and print your form. Make sure the bar code prints.

Electronic Data Interchange [EDI] Application PART B [for Providers billing directly]

  1. Select your State
  2. Click the drop down and select Start or Change Electronic Billing Method
  3. Check the box if you wish to receive your Remittance Advice Electronically
  4. Select Direct using a vendor
  5. Leave the Submitter ID box blank
  6. Enter your Business/Practice or Provider Name
  7. Enter your demographic information
  8. Enter the name and number of the main contact person for your office
  9. Enter your Fax number and E-mail address
  10. Enter your Medicare Number, NPI, and Tax ID
  11. New Applicants skip the section for Method of Interchange as this is only required if using an existing Submitter ID or reactivating the ID
  12. If using a Billing Service, enter their information at the bottom of this page
  13. Complete the last page and print your form. Make sure the bar code prints.

1. Select your State
2. Select the option for Billing Service
3. Choose the line of business that you will be billing for
4. Indicate with the drop down if you are a New or Existing Billing Service with Cahaba EDI
5. New Applicants should leave the Submitter ID box blank
6. Enter your Business Name and Tax ID
7. Enter your Demographic Information
8. Enter the name, number, and e-mail of the main contact person for your office
       a. Use your contact information for the Technical and Marketing sections
9. Check off In-House Software and enter the following
       a. AXIOM Systems, Inc - Vendor Name
       b. 602-439-2525 - Phone
       c. SolAce EMC Version 4.0 - Software Name/Version
10. Select ANSI 837 format 5010
11. Select ANSI 835 format 5010 for Electronic EOB's
12. Please complete the 276/277 section if you would like to apply for 276/277 (claim status) transactions
13. Please complete the last page with your information and print your form
       a. You will be the Network Service Company since you are a direct submitter.

NOTE To Billing Services: Once you have been assigned a Billing Service ID, your Providers will have to complete the Regular Part A Enrollment form and state that you are their billing service in their application.

Submitting your Forms

It is recommended that you keep a copy of all the forms you will be submitting for your records. Please fax the enrollment forms to:

It is very important that you complete and return the entire enrollment packet as described above. Incomplete packets will not be processed and will be returned to the submitter.

Waiting for a Response

Once the complete provider enrollment packet has been received, the documents will be processed. Processing will take approximately two weeks from the date of receipt.

After processing, AXIOM Systems will notify you to begin filing claims electronically. Usually, Cahaba GBA will fax a confirmation to the number listed as the contact on the form. If neither confirmation nor a returned packet is received after two weeks, please call the Cahaba GBA Support Center at 1-866-582-3253.
